I choose First Blood for my first match, but when I choose LMS or Triple Threat for my next match, does that mean that I can only choose from that sequence of matches, or can I still choose ambulance matches later on?

If you choose LMS or triple threat, that only means that you will have Last ride & Casket match/Three way dance available for you to unlock next time. Doesn't mean you can't go for HardCore and then Ambulance match. You can pick any sequence of matches you want.
